What Penis Thickening involves
Girth enhancement increases the circumference of the penis by adding volume in the layer between the skin and the erectile bodies, evenly around the shaft. It is chosen by men who are satisfied with length but would like more thickness, and it is the component of penile enhancement with the more predictable result. Two methods are offered: transfer of your own fat, harvested by liposuction from the abdomen, or hyaluronic acid filler injected in clinic.
The honest numbers: fat transfer typically adds 1–2 cm of circumference once the fat that survives has settled — a proportion, often a third to a half, is absorbed in the first three months, and the surgeon places volume with that in mind. Hyaluronic acid filler adds a similar amount immediately, is reversible with an enzyme, and lasts 12–18 months before it is absorbed and needs repeating. Neither changes length or erectile function.
What is deliberately not offered: permanent fillers such as silicone oil or PMMA, and dermal-matrix grafts placed around the shaft. These have a history of hardening, migration, infection and disfigurement that cannot be reversed. Reversibility and your own tissue are the safety principles here. If a clinic anywhere offers a "permanent" injectable for girth, that is a reason to walk away.

Techniques and options
Autologous fat transfer
Fat is harvested by gentle liposuction from the lower abdomen, purified by decanting or gentle centrifugation, and injected through two tiny ports at the base in many fine passes around the whole circumference of the shaft, beneath the skin and above the erectile bodies. The glans and the urethra are avoided. Even distribution on the day and massage afterwards are what prevent lumps.
Hyaluronic acid filler
A firm hyaluronic acid gel is injected in clinic under local anaesthetic cream and a penile block, in the same plane and pattern, over 30–45 minutes. Results are immediate, adjustable and reversible with hyaluronidase; the gel is gradually absorbed over 12–18 months. Volume is priced per millilitre and several millilitres are typically used.
Choosing between them
Fat is your own tissue and, for the portion that survives, permanent; it needs liposuction and a short anaesthetic, and some absorption and irregularity are possible. Filler is quick, reversible and non-surgical but temporary and repeated. Many men start with filler to judge the change and move to fat later.
Staged transfer
Rather than over-filling in one session — which raises the risk of lumps and poor survival — a second, smaller fat transfer after 6 months can add girth if wanted.
What is not used
Permanent synthetic fillers (silicone, PMMA), paraffin, and circumferential grafts are not offered because their complications are serious and irreversible.
What happens on surgery day

Recovery timeline
Days 1–3

Results — and their limits
Typically 1–2 cm more circumference, measured against your baseline, evenly around the shaft in both the flaccid and erect states. The change is noticeable to you and your partner; it is not dramatic.
Fat that survives the first three months is permanent and behaves like the fat elsewhere. Filler lasts 12–18 months and is repeated when it fades, or dissolved if unwanted.
Length and erectile function are unchanged.
Slight irregularity in the early months is common and settles with massage; a touch-up after 6 months can even out or add volume.
