What Buttock Reduction involves

Buttock reduction makes the buttocks smaller and less projected while keeping — or improving — their shape. It is asked for by patients whose buttocks and hips are out of proportion with the rest of the body, who struggle to find trousers that fit at the waist and hips at the same time, or who have gained disproportionate fat in the area after weight change or pregnancy. It is also the operation for correcting an over-filled or asymmetric BBL done elsewhere.

The core technique is liposuction, but liposuction of the buttocks is different from liposuction anywhere else. The buttock’s shape depends on fat, and the fold beneath it depends on ligaments that must not be disrupted; removing fat carelessly leaves a flat, sagging or dented buttock. The surgeon reduces volume from the upper, outer and side areas that create width, preserves the central mound and the fold, and blends into the hips and thighs so that the reduced buttock still looks like a buttock.

When the skin is loose — after large weight loss, or when a large reduction is planned — liposuction alone would leave sagging, and a buttock lift with skin removal is added. The trade is a scar along the upper buttock or in the fold beneath, hidden by underwear. What buttock reduction does not do: it does not change bone width, and it does not treat cellulite.

Who it suits — and who it does not

Good candidates

  • Buttocks and hips disproportionately large for your frame, with firm skin — liposuction alone

Techniques and options

Shape-preserving liposuction

Through ports in the gluteal crease and at the hip, fat is removed from the zones that create width and excess projection — the upper outer buttock, the sides and the "banana roll" beneath the fold — while the central buttock and the fold ligaments are left intact. Laser or power assistance helps in fibrous tissue. The result is a smaller buttock with a preserved contour.

Hip and thigh blending

The hips, outer thighs and lower back are treated in the same session so that the reduced buttock flows into the surrounding areas. Reducing the buttock alone leaves the hips looking wider by comparison.

Buttock lift with skin removal

When skin is loose, a crescent of skin is removed along the upper buttock (the scar sits at the bikini line) or in the fold beneath the buttock, and the remaining tissue is lifted and tightened. Combined with liposuction, this both reduces and re-shapes.

Fat transfer for shape correction

Paradoxically, a small amount of fat may be transferred into a hollow or dent — after an uneven BBL or in the hip dips — to restore a smooth outline while overall volume is reduced.

Compression and positioning

A compression garment worn for 6 weeks controls swelling and helps the skin adhere. If skin removal is included, sitting is restricted early to protect the scar.

What happens on surgery day

    Assessment before travel: photos from the back, sides and front, standing; weight history; previous fat transfer or surgery in the area; and the proportion you want. The surgeon plans liposuction zones and whether skin removal is needed.

    Admission on the morning of surgery after fasting. Blood tests, ECG and anaesthetic review; compression stockings and an anti-clotting injection.

    Markings standing up: zones for reduction, zones to preserve, blending areas at the hips and thighs, and any skin excision pattern.

    Under general anaesthesia lying face down, tumescent liposuction of the planned zones with blending; if planned, the skin excision and lift with layered closure. Drains are used when skin is removed.

    Compression garment fitted. Three nights in hospital lying on the front or side, with pain relief, anti-clotting injections and assisted walking from the first evening.

    Discharge to the hotel on day 3. Clinic visits for drain removal (if used), wound checks and the fit-to-fly report.

Recovery timeline

Days 0–3 (hospital)

Soreness and bruising like a heavy workout after liposuction; more tightness if skin was removed. Fluid leaks from ports for 24–48 hours. Lie on the front or side; walk short distances.

Days 4–10 (hotel)

Drains out if used; showers from day 3 with any incision protected. Sitting on a cushion for short periods only if skin was removed. Fit-to-fly report.

Weeks 2–3

Desk work resumes with a cushion. Bruising fades; the area feels firm and lumpy as the tissue heals — normal. Garment day and night.

Weeks 4–6

Results — and their limits

A buttock in proportion with your frame, with the width and projection reduced and the natural mound and fold preserved. Trousers fit at the waist and hips at the same time.

Removed fat cells do not return; the remaining fat responds to weight change. A stable weight keeps the result.

Liposuction areas may look uneven for the first months and smooth out with massage and time. Skin with good tone retracts over 3–6 months.

If a lift was included, the scar is hidden by underwear and fades over 12–24 months; small revisions of scar ends are occasionally wanted after a year.

Risks and how they are managed

Every operation carries risk. These are the ones that matter for this procedure, and what is done to reduce, spot or treat each.

Sagging or a flattened, unnatural shape

Prevented by preserving the central mound and the fold ligaments, reducing from the width zones only, and adding skin removal when the skin is loose. This is why buttock liposuction is a shaping operation, not a volume race.

Contour irregularity, dents or asymmetry

Even technique, blending into hips and thighs, comparing sides on the table; massage and time resolve most early irregularity, and small touch-ups are possible after 6 months.

Seroma

Compression garment, drains when skin is removed, limited activity; drained with a needle if it forms.

Wound-healing problems with a lift

Scar placement under the bikini line or in the fold, moderate tension, non-smoking, limited sitting; openings heal with dressings.

Numbness of the buttocks and hips

Common for weeks to months; resolves.

Frequently asked questions

Will my buttocks look flat afterwards?
Not if the operation is done as a shaping procedure: fat is removed from the zones that create width and excess projection, and the central mound and the fold are preserved. A flat result comes from removing fat everywhere, which is avoided.
Will I need a scar?
Only if the skin is loose enough to sag after fat removal. Then a buttock lift with a scar under the bikini line or in the fold is added. Firm skin needs liposuction ports only.
Can you fix a BBL that was over-filled or uneven?
Yes. Volume is reduced from the over-filled zones and, where needed, a small fat transfer smooths dents. Previous surgery makes the tissue more fibrous, so the plan is individual.
How much smaller can I go?
Enough to bring the buttock into proportion with your frame — the surgeon plans against your photos and measurements rather than a percentage. The shape sets the limit.
Will the fat come back?
